Workplace incident records · Not legal advice

Turn what happened at work into a clear, factual record.

Answer a short form about one incident. We turn it into a neutral, dated record, then a documentation assistant helps you fill in the gaps and write it up clearly, ready to keep or bring to HR.

What you entered · example

On Oct 6, right after my break, my shift lead came to my station and said loudly, "You're too slow, go home if you can't keep up." People on the dock heard it. Dana was next to me. It has happened before. I think he's been targeting me since I asked about my hours.

What it does

  • Organizes your account into a dated, chronological record
  • Keeps your words neutral and factual
  • Helps you draft a statement or email you choose to send

What it won't do

  • Give legal advice or represent you
  • Decide whether anything was unlawful
  • Cite laws or labor codes

How it works

From a rough account to a record you can stand behind.

  1. 01

    Answer a short form

One incident: what happened, who was there, when, where, any evidence, and what changed at work.

  2. 02

    Get a starting record

Your answers become a neutral record, with opinions, motive guesses and heated wording taken out.

  3. 03

    Refine it in a chat

Add details, fix dates, and draft a neutral statement or email you may choose to share.

  4. 04

    Save, print, decide

Review and correct everything, then save it as a PDF. You decide what to share and with whom.

Before you start

What to have ready

You don't need all of it. Write down what you remember; the assistant will point out gaps later.

Don't enter Social Security numbers, bank details, or medical information.

  • Who

The people involved and anyone who saw it.

  • What

Actions and exact words, as a camera would have recorded them.

  • When

Date, approximate time, and how long it lasted.

  • Where

The location, or the app or platform it happened on.

  • Evidence

Emails, texts, schedules or witnesses, and where they are kept.

  • Impact on work

Changes to hours, duties, pay, schedule, or discipline.

Privacy

How your words are handled

Plainly, with nothing buried in a policy page.

  • Not logged

Your answers are never written to server logs or analytics.

  • Encrypted, then deleted

Records, chats and drafts are encrypted and deleted automatically 30 days after your last activity. Delete sooner anytime.

  • No account, one private link

Anyone with your link can open your record, so keep it to yourself.

  • Who else sees it

The form provider keeps its own copy under its policies. Google's Gemini processes your answers to write the record and chat replies.

Write it down while it's fresh.

The details fade fastest in the first few days. Ten minutes now gives you a record to work from later.
